Case Manager (Homeless Services)
Job Description
Job Summary
The Case Manager provides intensive case management and clinical engagement to participants experiencing a housing crisis and homelessness. The primary goal is to promote housing readiness, facilitate access to housing assistance and rental subsidy programs (e.g., Section 8, EHAP), and secure permanent housing placement for families and individuals. This role requires extensive coordination with internal LAFH Programs and external systems.
Key Responsibilities
- Provide intensive case management, including crisis intervention, supportive counseling, and development/implementation of individualized housing plans and participant budgets.
- Conduct initial intake and assessments to collect comprehensive information (functional, housing, health, employment) to develop a case plan.
- Facilitate housing readiness and securing permanent housing by assisting with housing searches, applications, and the lease-up process.
- Connect participants to and advocate for services (health, mental health, substance use disorder) and assist with obtaining income and benefits (e.g., SSI, Medi-Cal, unemployment).
- Maintain accurate and complete case files/records, documentation, and case notes in physical files and the CHAMP system.
- Conduct bi-monthly home visits after a client is housed and assist with "warm" hand-offs to the housing stabilization team.
- Provide direct field-based services at identified motels, interim housing locations, and various environments (including street outreach, home visits, shelters, and potentially jail settings).
- Coordinate with landlords and community partners to address issues and barriers to permanent housing.
- Be available to assist when participants are experiencing an immediate need, crisis, or emergency.
Job Requirements
- Education: Bachelor’s Degree (Social Work or related field) OR equivalent related job experience (3 years). A Master's Degree is a plus.
- Experience:
- At least 2 years of experience in case management in re-entry, housing, benefits, or job development (4 years required if no degree).
- At least 2 years of experience working with the target population (homelessness, substance abuse, mental illness, formerly incarcerated, etc.).
- Certifications/Licenses:
- Valid California driver's license, auto insurance, and reliable transportation (required for travel).
- Ability to obtain and maintain CPR/First Aid Certification.
- Ability to pass post-offer Tuberculosis (TB) clearances.
- Knowledge/Skills:
- Knowledge of case management services and clinical approaches: Trauma-Informed Care, Motivational Interviewing, and Harm Reduction Models (preferred).
- Knowledge of tenant/landlord law and housing assistance programs (CalWorks, EHAP, Section 8).
- Exceptional ability to connect and engage with diverse populations.
- Excellent verbal/written communication, organizational, and time management skills.
- A passion for ending homelessness, demonstrating empathy and a non-judgmental attitude.
Additional Information
Pay Range: $15-$85
